We would like to thank Drs. Helia Hemmasian and Atousa Hakamifard for their valuable comments. We agree that the diagnosis of cerebral venous sinus thrombosis needs high degree of suspicion and the focus of investigation must be based on the given clinical context.1 Hence, we conducted all relevant tests based on the clinical situation. After all, we should not miss the elephant in the room.
